Annual Forms
In order to participate in activities, events, retreats, and missions with Hayes Barton Youth Ministries every student must submit the online Student Information Form and the Medical and Liability Waiver. These forms only need to be filled out once per school year, though there may be other forms specific to certain events and activities throughout the year.
